Measurements / Vitals Intake

After demographics are created, you may want to pre-populate the chart. Or simply enter vitals at an appointment as part of the chart. The intake form is used to load common measurements and CPP data from the appointment and caseload screens.

Intake Form

1. The intake form is used to load common measurements and cumulative patient profile data from the appointment and caseload screens.  Simply click on the “In” link next to the patient’s name
2. Fill in the required information
3. Scroll to the bottom of the form when you are finished and select Submit

The data is stored in the chart’s Measurements.

*Note: For the “In” (intake form) link to show up on appointments, it may need to be enabled in Oscar Properties (contact your Oscar Service Provider).
