Initially 200 U/kg (max 18,000 U) qd for 30 days, followed by 150 U/kg qd up to 6 months.
No specific dose advice for CrCl ≤30.
Dose reduction needed if platelets 50-100, drop 2,500 U if 46-82 kg, and 3,000 U if ≥83 kg.
Stop if platelets ≤50.

Tinzaparin:
Dose 175 U/kg qd for up to 6 months.
Max dose 18,000 U qd.
Use with caution if CrCl ≤30.
Stop if platelets ≤100.
