Agreement from family physician/NP
Resides in the Chilliwack, Harrison, Agassiz, or Hope community
Age 65 years and older (or younger on a case-by-case basis)
Medically complex issues
Multiple co-morbidities
Diagnostic uncertainty or atypical features of dementia
Management issues that are difficult to resolve
Exclusion Criteria:
Known to be actively suicidal
Client in acute care
Lives in Residential Care
