Requisition for Sleep Disorder Consult (Form B) Referral Form

This eFORM was originally paired with the HSAT (home sleep apnea testing) eFORMS which essentially removed all the advertising from the various respiratory support companies and genericized their referral forms.  If your patient does not qualify for HSAT, then you are to refer for formal sleep consultation or polysomnogram (PSG), using Form B.   There was considerable delay getting Form B finalized, and this is just the latest draft!  Only one site has requested the Form B format.  You are able to use the existing polysomnogram clinic referral forms if you wish.

The format is similar to the HSAT forms.
