Initiating Advance Care Planning....
If you became ill - from disease or accident - there may come a time when you cannot think or speak clearly for yourself. If that happens, you will need someone to speak on your behalf, to help doctors or healthcare providers make medical decisions for you. Who would you most like that person to be? Who do you trust? Who knows you best? Please help that person by considering the following questions:
What gives you joy? What provides your life with meaning and fulfillment?
Examples: singing, going to work, cooking food for my family
If you were to become ill, what would be important for others to know about you?
Examples: I am claustrophic; I have already completed an ACP plan; I wish to donate my organs
If your health were to deteriorate, what are your most important goals? What would be most important for you to accomplish?
Examples: meet my grandchildren; attend my daughter's wedding
Do you have any spiritual or religious beliefs that would affect your decisions about the types of medical treatment you may want or not want? What are they?
Examples: I consult with my pastor for important medical decisions; I would refuse blood transfusion
What are your biggest fears or worries about your health and medical condition?
Examples: having to ask others for help; being in severe pain

You are always the decision maker when you are
capable
of understanding your medical problem.
The Courts may appoint a "COMMITEE" to make these decisions in special situations.
In the event that you do not have a
representative
to help you make health care decisions,
Legislation
does allow health care providers to choose a
Temporary Substitute Decision Maker (TSDM)
to make urgent medical decisions on your behalf, if you are not capable at the time.
If you wish to specifically choose your representative, please consider completing a
representation 9 agreement
for them.
When your representative is aware of your advance care plans, understands your values, beliefs and wishes, he or she would be best prepared to speak on your behalf when asked to make health care decisions for you. See other page. →
