BC At Home HPV Requisition CCSL

For BC's Cervical Cancer Screening Lab (CCSL) Cervix Self-Screening program, which issues HPV swabs (PCR for HPV DNA) to patients directly. Some sites are doing these tests in office or providing kits to their patients who attend for scheduled PAP tests - for those patients you can use this eForm. Include collection date on both the requisition and specimen tube.
