INSTRUCTIONS:

To convert pounds to kg please type the value in pounds without any suffix (no "lb") and double click on the box.

To convert (Height) feet and inches to cm please type in the style 5' 9" and double click on the box.

Clicking the "Write & Submit" or "Submit" buttons will store the Ht, Wt, and BMI in the "Measurements" section for use in other areas of OSCAR.

Both metric and imperial measurements will write to the eCHART, but only metric values will save in "Measurements".

If you have started typing a chart note before opening this eForm, and you click the "Write & Submit" buttons, then you will be notifed that your existing chart note has not been saved.  You will be asked if you wish to replace it.  Click "Cancel" when this happens.  You will still be able to save your chart note later after you have submitted and closed this eForm.
