Fraser Health Pulmonary Diagnostics Referral

Created originally by John Robertson at the request of the Division of Family Practice in Chilliwack. Referral to Pulmonary Diagnostics in Fraser Health. There is redundancy in the eForm to allow for multiple work flow options. The patient handout can be printed from a link or by using one of the print buttons at the bottom of the page. The fax numbers can be copied when clicked and the ability to fax from OSCAR is also coded in. Specific portions of the second page are also available in the side menu.
