Dalteparin:
Dose 200 U/kg (max 18,000 U) qd, or 100 U/kg bid.
No specific dose advice for CrCl ≤30.
Dose reduction needed if platelets 50-100, drop 2,500 U if 46-82 kg, and 3,000 U if ≥83 kg.
Stop if platelets ≤50.
Enoxaparin:
Dose 1 mg/kg bid, or 1.5 mg/kg qd.
Reduce dose to 1 mg/kg qd if CrCl ≤30.
Stop if platelets ≤100.
Nadroparin:
Dose 171 U/kg qd (max 17,100 U/d), if increased risk of bleeding, reduce to 86 U/kg bid (max 17,100 U/d).
Decrease dose by 25-33% if CrCl 30-50.
Do not use if CrCl ≤30.
Stop if platelets ≤100.
Tinzaparin:
Dose 175 U/kg qd (max dose 18,000 U qd).
Use with caution if CrCl ≤30.
Stop if platelets ≤100.
