Standard Outpatient Bone Densitometry (BMD) Requisition

This Bone Density eForm is based on the PDF standardized for BC use. The locations, however, are Lower Mainland sites, including hospitals. There is an "osteoporosis calculator" that relies on age and current weight (kg). If the weight measurement is not current, you add the current weight to the "recalculate" field and it will push the input to the WT measurement on submit.
