CMPA: Governing Law and Jurisdiction Agreement eForm

Updated 2023/12/01/ 09:51 - the last instance of current_user ( current_user6 ) had the database tag patient_name not current_user. Now corrected.

This has a 2014 date stamp, but may still be relevant if you care for patients regularly, who live in another jurisdiction (e.g. USA). CMPA will cover you, only if the litigation takes place on Canadian soil.
