Covid Monitoring Record e-form and Report by Template to find active records.

The tool may also be used as a monitoring template for patients discharged from hospital after initial management of COVID. The eForm links to best practice information for assessing and managing COVID-related illness that has been curated by our Department and made available on the Hamilton Family Medicine (HFAM) web-site.

In addition to the eForm itself, we have included two key features that enhance workflow:

option to send automated ticklers to prompt certain follow-up tasks based on what is selected or entered on the eForm, and
a Report by Template designed to capture the active caseload being monitored at any one time using the eForm.

To change where the tickler is directed:
Currently this e-form will create ticklers for the internal user System, System. This can make it easy for more than one user to follow the requested tickler. If this does not work for your clinic, change the provider number in the javascript function: sendTickler(...) Replace the "-1" with the provider you prefer.
