Fraser Health Surgical Booking Form
For surgeons operating in Fraser Health. This form is tuned for Obstetrics and Gynecology but can be (has been) modified for other specialties. The original form is from 2014. It has been updated 2023/Aug/22 as the original version had components that did not work in Chrome. In the process of updating the eForm, all the more recent code improvements were added - including an accordion side menu, Required fields, parsing the date to the correct format, and the latest Xbox and Radio box code.
