RBT for List of Emails for Patients Seen Within the Past 36 Months

Reports emails for patients with Active status (AC) who have been seen within the past 36 months.

This script uses the BC billingmaster table. It can be adjusted to use the appointment table instead which would make it work for both ON and BC.
